To offer a verified Continuing Education (CE) program in Hawaii, you must apply via the Department of Commerce and Consumer Affairs or via the HVMA below. Please allow up to 2 weeks for processing.
To apply via the HVMA, please submit the following documents to ce@hawaiivetmed.org:
- Program outline including time schedule
- Curriculum vitae (CV) of all lecturers
Fill out the Program Sponsor Agreement for CE form here or below:
A $25 application fee must be submitted via PayPal or check (payable to Hawaii Veterinary Medical Association and mailed to PO Box 61309 Honolulu HI 96839-1309; check processing may delay approval by an additional 2 weeks). To qualify for the reduced member rate of $20, a current HVMA member must be listed as the Responsible Contact on the above form.
